The Chronic Disease Self-Management Program is designed to help people with chronic disease manage their condition and begin living more active and fulfilling lives.
The Campbell County workshop will take place on Wednesday, March 18 through April 22, from 12:30 to 3 p.m. The Florence workshop will be on Monday, March 16 through April 20 from 1 to 3:30 p.m.
Participants will learn how to:
• manage symptoms;
• set weekly goals that are achievable;
• relax and handle difficult emotions;
• work with your doctor;
• improve communication; and
• work exercise into your schedule.
This program has significantly improved the participant’s lives by helping them with:
• have less fatigue;
• have less disability;
• improved communication skills;
• have 0.8 fewer days in the hospital;
• increased minutes of exercise; and
• have more confidence in ability to manage chronic condition.

These workshops are free to the participants. Each participant will receive a book and a CD.
